ABOUT THE PAYMENT OF PREMIUMS TO ONYXIE

With added Recruitment Incentive – Monthly Recurring Income offered by Onyxie.

The contract, the Rules, Terms and Conditions stipulates that requests for Additions, Cancellations, Amendements Changes, increase of covers, Etc… must be in writing making use of the supplied LATEST official request form which once completed, signed and dated must be supplied,
by faxing it to Onyxie ON or BEFORE the 10th of the specific underwriting month: Onyxie is thus enabled to capture and activate the received request.

If NOT received on or before the 10th by Onyxie:
The request shall be captured and activated only the following underwriting month.

On the 11th or before the 15th, Onyxie by phone and SMS contact all entities
and indicate to them the correct total premium amount PAYABLE TO ONYXIE.
The total premium amount indicated is payable to Onyxie ON or BEFORE
the 20th of the specific month of underwriting or the 19th
if the 20th is a Sunday or public holiday.

A five (5) days grace is given but if on the 25th, the total premium amount is not paid: The underwriting IS AUTOMATICALLY SUSPENDED FOR ARREARS
And if by the 15th of the following month still not been paid: The underwriting IS IMMEDIATELY CANCELLED AND LEGAL STEPS AUTOMATICALLY INSTITUTED.
The contract also stipulates that to CANCEL the underwriting: The entity must give, in writing, ONE CALENDAR MONTH NOTICE (30 days)
DATED on the FIRST DAY of the 30 days notice, and, related to the 30 days cancellation notice: the total premium amount must be paid.
